Once I have your information, I’ll e-mail you back to set up a 15-minute individual phone call.

Our first session begins on March 11th, 2011 at 9:30 EST.

If you are calling by conference line, I will give you the conference number by March 9th. If you are participating by Skype, I will give you the Skype name by March 9th as well.

Each of you agrees not to duplicate any part of the book or the Journal. If you plan to use the book and journals with clients, they’ll need to purchase their own. Any materials I send you during the course are copyrighted and all client-related information is confidential.

I hope to create a therapist’s manual and/or training package, partially based on our work together. I ask that you allow our sessions to be transcribed and that you consent to allow audio and digital tapes to be made for the purpose of training mental health professionals and/or doing clinical research. The recordings will not contain your full name or other direct identifiers.

When you’ve finished the course, you’ll be eligible to be listed on my website as a therapist who has completed a course of training with me. For liability purposes, a disclaimer, similar to the one that you see on the APA referral site, will appear on the bottom of the page; and I’ll ask you to sign an indemnification agreement.
